If you are sick with COVID-19, please complete the secure voluntary disclosure form so Polytechnique can collect data required for pandemic management purposes. Please include any relevant information. After completing the form, don’t forget to select the Soumettre (Submit form)  button to transmit the form. 

If you are sick with COVID-19 AND you must miss a graded activity (midterm exam, lab, practical work, oral presentation, class, final exam, or other) during the period which you are incapacitated, you must also complete the Formulaire de motivation d'absence du Registrariat (Absence justification form issued by the Registrar’s Office). This form will be used to determine whether your request will be accepted or rejected.
